Consent Form

I hereby grant Zealous Vibes LLC (the “Producer”) the unrestricted right to record, use, edit, distribute, and publish my name, voice, likeness, and any other contribution I make during the podcast episode for commercial and promotional purposes, in any media, now or hereafter known, including but not limited to audio and video formats.

This grant of rights includes, without limitation, the rights to:

    •    Use my voice, likeness, and statements in the episode.

    •    Edit, adapt, and create derivative works from the recorded content.

    •    Distribute and share the content on all platforms, including but not limited to streaming services, social media, and websites.
